After years of building an APRN-led mobile wound care practice across Tyler and East Texas, we are bringing the same model — advanced wound treatment delivered at the patient's location — to families, facilities, and referral partners across the DFW metro and beyond.

Our position has never changed: quality wound care should come to the patient, not the other way around. Every week, patients with chronic wounds miss appointments because transportation is hard, mobility is limited, or a wound clinic visit means four hours away from home. Missed visits mean missed healing.

Why the Metroplex, and Why Now

The Dallas–Fort Worth–Arlington metro is the fourth-largest in the country and added roughly 123,557 residents between July 2024 and July 2025, reaching about 8.48 million people (Fort Worth EDP, WFAA). Growth of that scale drives demand for post-acute and chronic-condition care faster than brick-and-mortar clinics can absorb it.

Meanwhile, the underlying wound burden is substantial. Chronic wounds affect roughly one in six Medicare beneficiaries — about 10.5 million people — at an estimated cost of $22.5 billion per year (Human Wound and Its Burden: Updated 2025 Analysis). In Texas specifically, about 3.1 million adults (13.2% of adults) have diagnosed diabetes (American Diabetes Association), and roughly 12% of people with diabetes develop a diabetic foot ulcer (CDC). Pressure injuries are common in older adults as well, with pooled prevalence of about 11.6% for any stage (Pooled prevalence review, 2023).

Put simply: DFW has a large and growing population of people who need consistent, specialized wound care — and many of them cannot easily travel to get it.

What Is Mobile Wound Care?

Mobile wound care means a licensed wound care provider evaluates and treats the wound wherever the patient already is. No transport van, no waiting room, no shuffling between three appointments in one week.

At Vitality Mobile Wound Care™, visits happen in:

·        Skilled nursing facilities and nursing facilities

·        Telehealth follow-ups when clinically appropriate

Advanced clinical visits are performed by Advanced Practice Registered Nurses (APRNs). Follow-up and treatment-continuity visits are performed by our nursing team under the supervision structure required by Texas rules and payer policy. That staffing model is deliberate — it keeps advanced decision-making with advanced practice clinicians while still allowing frequent, hands-on wound treatment between advanced visits.

Conditions We Treat Across DFW

Our clinicians focus on wounds and only wounds. Common referrals include:

·        Diabetic foot ulcers (DFU) — offloading strategy, infection surveillance, vascular referral coordination

·        Venous leg ulcers (VLU) — compression therapy and edema management

·        Pressure injuries — Stage 1 through unstageable, including facility-acquired injuries

·        Surgical wound dehiscence — post-operative wounds that reopened or stalled

·        Traumatic and skin-tear wounds — common in frail and anticoagulated patients

·        Chronic non-healing wounds — wounds that have failed weeks of standard care

·        Ostomy, continence, and complex wound needs — supported by WOCN capacity on our team

Advanced Modalities, Delivered at the Bedside

Bringing care to the home does not mean bringing less care. Our clinicians deliver:

·        Selective and surgical debridement

·        Advanced dressing selection and negative pressure wound therapy (NPWT)

·        Cellular and tissue-based products (CTPs / skin substitutes) for eligible wounds

·        Low-frequency, non-contact ultrasound therapy (MIST/UltraMIST)

·        Compression systems and offloading

·        Nutrition, glycemic, and vascular coordination with the patient's other clinicians

Coverage rules matter here. Not every wound qualifies for every product. For CTPs and advanced modalities we document wound measurements, comorbidities, and a documented course of standard wound care before advancing therapy — consistent with Medicare local coverage determinations (CMS Medicare Coverage Database). If a therapy is not covered or not clinically indicated, we say so.

Why Home-Based Wound Care Matters Clinically

Chronic wounds do not fail from lack of effort — they fail from gaps in follow-up. In one tertiary-center cohort of 444 patients, lack of follow-up care was an independent predictor of hospital readmission (hazard ratio 2.39) (retrospective cohort study). And in a nurse-driven wound program, patients receiving consistent weekly wound care with clear multidisciplinary communication had very few unplanned readmissions (Journal of Vascular Nursing).

Removing the travel barrier is one of the most direct ways to protect that follow-up cadence. We cannot promise any individual outcome — no honest wound practice can — but we can promise consistency, measurement, and a documented plan reviewed at every visit.

How often will someone come out?

Is mobile wound care covered by Medicare?

Medically necessary wound care services are generally covered when documentation supports them. We verify your specific plan and benefits before we begin.

Do you treat wounds in nursing homes and assisted living?

Yes. Facility-based wound rounds are a core part of what we do, and we coordinate directly with facility nursing leadership.

What if my wound has not healed in months?

That is exactly the referral we want. Non-healing wounds need reassessment — vascular status, infection, nutrition, offloading, and modality selection all get re-examined.
